Netra tarpana therapy involves three stages; Purva karma, Pradhan karma, and Paschat karma.

Purva Karma

In this stage, the physician analyses the Prakriti ( body type)and vikriti ( morbidity). After a detailed diagnosis, the physician suggests Netra tarpana.

A) Pradhan Karma

The room chosen for Netra tarpana should be free from smoke and dust. The patient has to lie down on their back in a comfortable position facing upwards. For Netra tarpana, freshly made dough ( made with black gram flour) rings are placed around the eyes and filled with fragrant oils. Then lukewarm medicated ghee is poured on closed eyes up to immersion of eyelashes. Once the required ghee is applied to the eyes, the patient has to open and close the eyes at proper intervals to ensure the absorption of medicated ghee to the inner corners of the eyes. The ghee used for this treatment can reach the internal ophthalmic structures.

B) Paschat Karma

Once the therapy is complete, the medicated ghee drains out of the ring. Then the region of the eyes will clean with cotton. The next procedure is to wash the eyes with lukewarm water. Then medicated smoke is given to the patient to pacify the increased Kapha. Until the completion of the therapy, the patient should not take a head bath and should avoid contact with bright lights, extreme temperatures, etc. The patient has to stay in a clean environment during the treatment period.
